1. HOME LOAN APPLICATION CHECK LIST.-

Thank you for the opportunity to be of service to you.  Please gather the applicable items.
If you are unable to locate everything, please do not hesitate to submit the available items via EMail

RESIDENCY:

___ list of Addresses for the past 2 yrs (showing from MONTH/YR to MONTH/YR).

___ Land Lord names, phone #s, and mailing addresses for the past 2 yrs (if applicable).

EMPLOYMENT:

___ list of employers (name and address) past 2 years (showing from MONTH/YR to MONTH/YR).

___ 2 most recent PAY-STUBs.

___ most recent W-2s for the past 2 years.

___ most recent Tax Returns for the past 2 years (complete), and
IF SELF-EMPLOYED or IF YOU OWN MORE THAN 25% of a corporation:
-- complete Corporate Tax Returns or 1120S for the past 2 yrs, (YTD.P&L + Bal.Sheet)  
-- If going “Stated”: Occupational License for the past 2  years, or
                             Accountant Letter verifying Tax Return Preparation for at least 2 yrs.

OTHER INCOME:

-- Child Support, Pension, Disability, Social Security, etc. * current letters stating amount received.
     (Florida Residents call 800/772-1213 for Soc Sec Benefit Letter).

___ Documented Evidence of Earnest Money Deposit towards purchase  (e.g., canceled checks).  

CREDIT REFERENCES:

___  $195 application fee payable to Al Baillères . . . ($25 credit report, and $170 towards closing costs)

___ Photo ID and Social Security Card.

___ Home Rental Payment History: copy of front & back of the canceled checks for the last 12 months

___ most recent Year End Mortgage Statement (on Real Estate owned – appearing on Tax Returns).

___ if applicable:

-- copy of Divorce Decree & Separation Agreement.

-- Bankruptcy discharge. Schedule of Debts & Reaffirmed Accounts. Written Explanation.

ONCE YOUR OFFER TO PURCHASE IS ACCEPTED:

___   Purchase Agreement or Contractors Estimate.

___ Home Owners Insurance (flood-if applicable) * cover page outlining coverage,
and/or Home Owners Association Manager's name & phone # (on PUD or CONDO).

___ Title Insurance (schedule A & B only).

___ most recent Survey (preferably the original w/the raised seal & certified in your name).  

___ Trust Agreement (if property is vested in a trust)

___ most recent Appraisal (if available).
